Apply to Health Opportunities Fund


For the Health Opportunities Fund, the foundation ONLY accepts funding requests via its online application system. This system includes all stages of the application and reporting processes.

Before accessing the online system, please review the following tips:

  • Video tutorials. If you are a first time user, you may find the tutorials helpful. There are a video tutorial and written tutorials for creating an online account and for completing an application.
  • Check eligibility. Review the Eligibility Criteria to determine if you are eligible to apply for support from the foundation.
  • Draft in Word. Review or print out the application questions to refer to as you prepare your draft, then copy and paste your answers into the boxes online. This will make it easier for you to edit and finalize your information before submitting it. Please note: the system imposes character limits in each field.
  • Fax to File. You may need to scan in and upload certain documents to attach to your application. If you do not have a scanner, you may use the “Fax to File” tool. Click on the “Fax to File” link for detailed instructions. 


When you click the “access online application process button,” you will be asked to create an account with login name and password. Please create only one account for your organization.

If your organization has an account, please use the login credentials already established. If you do not know your organization’s username and password  or are unsure if your organization has an account, or if you have any other questions, please contact us.


While the foundation is not currently accepting unsolicited funding requests for the Health Opportunities Fund, organizations working with the dual eligible population in Montgomery County interested in exploring funding opportunities should contact foundation staff at 215-716-5400 or
